What You Will Do:

• Provide direct resident care services. Follow individualized plan of care for all residents.
• Administer medications following all standards of practice, state regulations, and policies, to include all required documentation, reporting, and follow-up.
• Perform resident assessments as required by the state standards/ regulations and facility policy, under the direction of the Director of Skilled Nursing.
• Observe residents for changes in condition and report any changes immediately to the supervisor and/or the Director of Skilled Nursing.
• Document all pertinent/relevant observations, communication with the resident’s responsible party, clinical procedures, and daily assistance/care by the end of each shift.
• Complete and maintain current individualized care plan for each resident.
• Check and record vital signs, weights, and blood sugars as assigned.
• Provide for or assist residents with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc. Encourage residents to participate in scheduled facility activities providing assistance in getting to activities as needed. Assist in the planning and implementation of resident activities as needed.
• Assist residents with transfers and ambulation as directed, using proper body mechanics.
• Respond to urgent calls as needed. Supervise direct care staff, ensuring that resident care and medication assistance is provided correctly and in accordance with policies and state regulations. Provide coaching and training to care staff as needed under the direction of the Director of Skilled Nursing.
• Perform other duties as assigned by Supervisor.

Supervisory Responsibilities:

• The position falls under the supervision of the Director of Skilled Nursing but is expected to perform independently and exercise good judgment. The position will assist with daily supervision and the direction of work of other associates.

• High school diploma or equivalent (GED) required. 
• Registered Nurse in applicable state required.
• Current CPR certification required.
• At least 2 years of related work experience required.
• On a daily basis, throughout the workday: Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, and fine manipulation. Must physically be able to lift up to 25 pounds unassisted.

What You Will Do:

• Plan, implement, coordinate, and evaluate programs of therapeutic recreational activities (including evening and weekend activities) for Assisted Living and Memory Care residents. Ensure programming meets minimum requirements under regulations governing relevant Senior Living communities. 
• Consults with other departments in implementing appropriate activities relating to the physical, emotional and social needs of residents.
• Assist in coordinating transportation and accompany residents on outings as scheduled.
• Develop and post a monthly schedule of member activities by the first day of the month. Maintain a record of activities for twelve calendar months at all times. Make records available to regulatory agencies upon request.
• Assist with the volunteer program to meet the program needs of the residents.
• Perform other duties as assigned by Supervisor.

• High school diploma or equivalent (GED) required.
• At least three (3) years of related work experience in the senior living industry or related field required.
• Current CPR certification preferred.
• Certified Activity Professional or equivalent preferred.
